When applying for program benefits through ACCESS WI, you need to include information to determine your eligibility.
To ensure a smoother, quicker application process, please have the following items ready before you begin:
- Social security number
- Date of birth
- Address
- U.S. citizenship or immigration status
- Housing and utility expenses
- Job information
- Income information
- Other income information like if you or someone in your household gets child support, unemployment, or Social Security
You might be asked to provide any of these items during the application process.

How to upload documents into ACCESS
You can provide these in the following ways:
- Scan the documents onto a computer and upload the scanned documents when the system indicates.
- Take a photo of the documents with a phone or camera, transfer the photos to the computer you are using and upload them when prompted.
- Tip: If you need help getting a photo to a computer, you can email the file to yourself and download it to the computer from your inbox.
SAFETY NOTE: If you are using a public computer to apply, be sure to delete the files from the computer after you have uploaded them.
